Procedure for enrolling a subject and allocating the treatment (allocation concealment procedures)
The allocation was assigned using sequentially numbered sealed envelopes. The research nurses were responsible for recruiting participants and assigning participants to groups. The research team responsible for recruitment was blinded to the methods of randomization until recruitment was complete.
